- UCCS VA Course Load Worksheet (signed & dated)
- ONE of the following VA Forms:
- VA Form 22-1990 (1st-time MGIB recipients)
- VA Form 22-1995 (If you have ever previously submitted a VA FORM 22-1990)
RETURNING UCCS MGIB STUDENTS
Fax (1-719-255-3108) or scan & email (vacert@uccs.edu) the following to UCCS VA Office:- UCCS VA Course Load Worksheet (signed & dated)
NOTE TO FIRST-TIME REGISTRANTS:
Verify that UCCS received your documents by calling 1-719-255-3253.
After verifying, mail original signed VA Form & a copy of DD 214 to the below address (mailing to this address typically reduces processing time over other regional VA offices). (Do not send your original DD 214!).
